Please complete this packet before your first appointment. It provides the information needed to begin counseling and explains the services, policies, privacy practices, and telehealth considerations we will review together.

How to complete your paperwork

  1. Download the blank fillable packet.
  2. Open it in Adobe Acrobat Reader or a compatible PDF viewer and complete all applicable fields.
  3. Review the acknowledgments, then enter your full legal name and the date in the electronic-signature section.
  4. Save the completed PDF to your device.
  5. Upload the completed packet through the secure VSee patient portal or follow the secure VSee instructions provided by Dr. Luster.
